Do you think there is something wrong if I still haven't recevied my order form?

I read in other forums that even two days ago some collectors received an order form.
One said that there are no more 2E CC Proof available... so if you ordered this coin you will probably not get it.
I think you will be able to order the other issues though.

I meant that I still haven't received my order confirmation, not order form.
I ordered roll Malta 2e CC 2012 that was issued in October.

You are right... and that's what I meant earlier too...
A lot of collectors received a few days ago confirmation with total shipping +postage.
When I said earlier order-form I meant confirmation.

1x 2E CC Proof and 1x BU set 2012.
I haven't received them yet... but I already paid...

I finally got contacted by Malta Bank!
I got allocated the roll Malta 2e CC 2012!
Finally, only took 9 months of waiting..

They contacted me 27.12, will soon make payment.
